How can I prepare for my upcoming child care subsidy/voucher appointment?

If you have just been offered a child care voucher (financial assistance to pay for child care), please gather the documents below. Your Family Specialist may ask for additional information but having these documents handy will help the enrollment process run smoothly. 

1. Valid Photo ID   

1. Proof of address within 90 days   

1. County issued birth certificate for all children in the household under 18 

1. Access to proof of your income (paystubs, other documents or appropriate supervisor or HR personnel)
